In order to bill your insurance, a doctor’s prescription is needed.

Though treatments will vary depending on your specific needs, generally plan an hour for each visit.

For your first visit, please arrive 15 minutes early to fill out paperwork. Bring the physical therapy referral from your doctor, a picture ID (driver’s license), and your insurance card(s). Wear comfortable, loose clothing such as shorts, t-shirts, or any athletic attire.
You will be evaluated by a licensed physical therapist. You will discuss your symptoms in addition to any other pertinent information such as activities that are affected by your condition. Your therapist will then take you through tests and measures including strength and flexibility testing and ultimately create your unique treatment plan.
Several factors determine the number of treatments necessary. Your clinical presentation and doctor’s recommendations are all factors. Your insurance coverage may also limit the number of treatments you are allowed. Your physical therapist can provide you an estimate of the frequency and duration of your treatment plan.
Almost all health insurance plans cover physical therapy. Contact your health insurance customer service by calling the phone number on the back of your insurance card, review your plan coverage online, or consult with your employer’s benefits manager to determine the extent of insurance coverage for physical therapy.
